If you have an LLC that is void of assets, never had assets, no ein # - is it a requirement to report it on the petition? I looked over the paperwork and do not see where it is a requirement given it is an empty shell.

The corporate structure is so easy to track down. A phone call or search online by the trustee will show it exists in a matter of moments.
I can't imagine it changes anything, anyway. If it is an empty shell, well, what good is it for anything.
Also, even though it has no ein, it probably does have stock, even if that has not been formally issued. Usually in the corporate charter, there is a designation of how many shares. THAT is something that has to be reported, at any rate.
