Date claim was incurred will be the date the acct was opened until Closed or to the date of last charge made, which ever is applicable.

Consideration for claim will say something like XYZ Visa/MasterCard Charges.

On our Sched F, only 1 acct had a box checked. Our Citi charge. The attny X'd off the box for "Disputed" and we don't know why. We never Disputed the acct. But Citi had passed that acct on to Collections. So maybe some conversation our attny had with one of the Collections Agents lead to the "Disputed" comment.

For dates you put the range of when opened to when closed (or last charge) - like Sinkingfast said. If you have a bill with just one date (like a medical bill), you would just put that down. Then you put what the bill is for; medical, credit card charges, furniture loan etc. You probably won't have any of the boxes checked. I would suggest you get the Nolo book if you are filing pro se. It gives really good instructions for the forms and extra tips on what needs to be filed when etc.
