chapter 13 plan

Hi guys,

i need a favor. can anybody show me a sample of a chapter 13 plan? the one that's been submitted or ready to be filed in court?

or can anybody point me to a some resources on the making of a chapter 13 plan? i wonder how much should be paid to the trustee, how will the payment be distributed, etc. my debt is primarily from the IRS and some credit cards. the IRS has already levied on my salary and 401K although it has not been implemented yet.
