US Trustee form lost. [Archive]

I received a letter from a US Trustee not to long ago, it said to fill it out and give it to my trustee at the 341 meeting. I just found out that my girlfriend accidentally threw it away, does anyone have any idea what the form is?

All I know is it had something to do about not being represented by an attorney.

I have no idea. What you need to do is call the Trustee's office and ask them for another one, or if it's available on-line.
