Your credit report is not the place to get accurate information about your student loans. Also it's not unusual for student loan lenders (even govt ones) to shift your account over to third party companies while your Ch 13 was active.

Now that your Ch 13 is done (CONGRATULATIONS!!!!), contact the lender directly. Ask for an accounting in writing for your loan since your Ch 13 filing showing all account activity month by month. That's how you can find out exactly where the lender says you stand now. If you disagree, then dispute their accounting using your own financial records.

Now that your Ch 13 is over, the lender expects you to start making full payments again immediately. When was your Ch 13 discharged? Have you made payments since then? (And by the way, please note that the minimum payment they request typically covers the interest only every month. Paying this loan off as quickly as possible should be a goal for your post-13 financial budget.
