    Certificate of Service (Amendment)

    Ok, I added a few creditors to my schedule F. I printed an "Amendment to Schedules" form that lists all affected creditors and my wife and I signed it and turned it into the clerk. (At the bottom of this form reads 'Certificate of service" - - i certify that a copy o this amendment has been served upon the trustee and the following parties affected thereby... (I listed the creditors here) then how they were notified and signed by my wife and I)

    Last week I received a letter requesting a "Certificate Of Service".

    After calling the clerk several times the case worker isnt in and wont be in until tommorow (Was told monday last week) So im at a loss until I can talk to this person and find out what they need.

    I have until the 30th to file this. What bites is my wife and I are seperated and lives 4 hours away. So it will be a task to get her to sign something else and get it in before the deadline. As I have said I have called and left messages for this case worker who is out of the office..

    Anyone have any ideas? On what they are looking for?

    I know there is a 'proof of service by mail' form that was optional to print w/ the form7.com software I used. Not sure if this is what they meant.

    If this is what they want, it includes the signature of someone verifying the notices were sent & the names & addresses of creditors involved. (The form I saw indicates it should be someone not on the petition.)
    Most of my information is from personal experience or HOURS and HOURS of online research. When you're searching online, keep in mind there is no guarantee that the info is completely up to date, and your situation is unique from anyone else's. Do your homework, and consult with an attorney so you can make an informed decision.

      (1) A separate matrix page is required for creditors being "added", a separate page for those being "deleted", and a separate page for those which require "correction". Indicate on the REVERSE side of each matrix page which category that particular page belongs in.

      (2) The matrix with Verification is a document separate from the amended schedules and may not be used to substitute for any portion of the schedules. IT MUST BE SUBMITTED WITH THE AMENDMENT


      This is whats listed on the courts website..
