    Mailing List Format Help!!!

    I'm filing pro se. I've gotten my information together, but I'm not clear as to how to format the mailing list other than indentations. Is there a formal header page? My court rules that this is filed as a seperate form, but I have no idea how to proceed.

    Can anyone show me?

    I don't know what state you are in but this site is for my state and gives guidelines for the mailing list. (In New Mexico it must be submitted on disk or CD). Since this is Federal I assume all states basically the same?

      Remember that most mailing matrixes are used by clerks to generate mailing labels...here in Oregon, they want it submitted on a floppy or CD in text only format (notepad) & all left justified, no more than 5 line....again, that would be what would fit on a mailing label....hope that helps!
