      Ok, who issued the notice.

Generally, Notice of Levy is the type of term the IRS uses. Has the IRS levied your bank account? If it is from the IRS, generally, there are instructions on the Notice of Levy about how to go about an appeal.

You didn't ask any questions. What do you want to know?

Who sent you the information subpoena, the bank or your creditor?

Was there a list of questions attached about what and where are your assets?

Since your creditor received nothing from the bank levy, it is now asking you to reveal the rest of your assets to them under oath and court order. If you fail to respond you will be held in contempt of court. If you don't appear for your contempt hearing, a warrant will be issued for your arrest.
