Creditor Matrix

So I'm filling in my Ch. 7 matrix and I have a question for all of you. I have an American Express bill (amongst many, many others) that a collection agency (Allied Interstate) is attempting to collect. When I list out the names and addresses of creditors, I need to include the collection company also, correct?
What happems to that tradeline on my credit report if I don't list the collection company? Does the Amex bill being discharged (assuming everything goes well) automatically wipe out the associated collection account from Allied Interstate?If I don't list Allied Interstate on my matrix or anywhere else in my BK papers, do I still owe them the money even if the original debt has been discharged?

I had to list the CC company as well as the collection agency for my accounts that are in collection. This assures that everybody is notified from the court and you don't risk the chance of the debt not being wiped out.
