Help! I cant find this form

You guys have been such a great help. Hope you can help me with this.

I am triple checking all of my schedules and statements. I have a list from the courthouse of all of the documents they need. They have 1 listed that I have not heard of. It is called "Statement of Current Monthly Income and Disposable Monthly Income". I have already done all of the schedules with that information on it, but this is a separate statement. I checked on my local court's website and cant find the form there. Not sure if this is a local form they are requiring or if it is required of everyone.

Someone please help. I am going to call the courthouse when they open but not sure if they will be able to tell me, as they are not very helpful.

Sorry, I forgot to mention that we are filing Chapter 7, in Virginia.

It's just the means test.

Seeing as you are doing a 7 it's the means test. If you were filing 13 it's a little different form.

Thanks. I thought at first it was the means test as well but the means test is listed separately. This is a total different form.

I am actually on the line with the courthouse now and they cant seem to figure out what it is either.(great!)

ok, they got it figured out. It is a misprint. That is for chapter 13 not 7. They just listed it under the wrong chapter.(whew) I find it hard to believe that I am the only one who caught that. It took them 16 min to figure that out.
