what does a post confirmation modification mean?

I have a cousin who has filed chp 13 within the last year and I told him about this forum. He wanted me to ask, one of his creditors has filed a post confirmation modification in which he has a hearing on jan 8. what exactly does this mean?

You will want to look at Section 1329 of the BK code.

Since the motion was filed by a creditor...the motion is probably related to how much the creditors will be paid, the schedule of payments, or the amount that will be distributed to that creditor.
