How do I know if the bank perfected my lien?

I purchased my condo originally in May 98 and financed it through the bank where I was working. Fast foward to May 2006, I refi through the same bank and I also obtained a home equity loan. I was lucky that I did that because if I hadn't, I'd lose the house because I would have to much equity in it to exempt it when I declare BK. So back to the point of my post.....I read about things that happen when liens are perfected (especially Minny's). I obtained a copy of the recorded deed from the county clerk. It is showing the original date of May 98. Now here's my question...does this lien appear to be perfected? The date does not reflect the refi, but the debt really didn't change hands. How can I tell?

Thanks for everybody's help in sorting out this maze of BK!!
