Question about listing creditors

A few of my bills I plan to list on my bankruptcy have went to collection agencies. When listing these on there do I use the current collection agencys address and info or the original creditors info?

Both. List the OC first and then any and all collection agencies below it. Don't list the amount of the debt more than once, tho. "See OC XXXX, above" should do it in the CA's amount columns.
