Need Help Listing Creditors for Matrix & Schedule F

I'm working on filing Emergency Chapter 7 pro se, and am confused about the creditors listing - some of my accounts have had a number of collection agencies involved, and I also find the original creditors' names shown differently (like American Express, American Express TRS Inc) along with several different addresses for the same company.

Do I list all the collection agencies or just the original creditor?
Do have to have the exact legal name of the creditor, or can I just pick one name and address to be covered?
Do I include my landlord, phone co., internet provider when I don't want to have any debt discharged with them?

You should list all - original and collection agencies.
I didn't list any of my utilities or regular bills and I made sure they were paid up as soon as I could.
It's a good idea to list more than one address if the creditor has more than one and you don't know which is the main address.
