Amendment Form

Hi All,

Well today I got a letter from the US Trustee asking for documentation on expenses, last 3 years of tax returns and W-2's and current pay stubs. I've gotten all my documentation together and do have a few things I've overestimated, some I've underestimated and a few I forgot all together. So, I want to file an amendment but can't find the proper form. Can anyone tell me where I can find one online for free? I tried the Washington State bankruptcy website, but don't find anything that says "Amendment". Any help would be much appreciated.

I believe you would use the same form that you filed with, but note it as 'amended' at the top/center of the page.
