Credit report dispute

I have two items on my credit report that are collection/charge offs that are not reporting as IIB or discharged. Which docs would I want to submit in my dispute? The discharge order is two pages and doesn't list creditors and account numbers.

You should only need to provide them with your file date and chapter #. I just disputed something in may with transunion and that's all I gave them. Got word yesterday it was deleted. One month after opening dispute.

I disputed a few items today that showed charged off, but were included in bk. I'll see if they get removed. A few of my husband's items are falling off in the next few months. Not sure if I should bother disputing. I don't want them staying on longer. Does anyone know if a dispute changes the date?
