    No, you don't put the "any" mortgage. You are only listing property and its encumbrances, so basically one "entry" per "address".

    Example:

    DESCRIPTION AND LOCATION OF PROPERTY
    123 Main Street, Anytown, FL, 90210
    LOT 10 IN BLOCK 1 OF HOMEVILLE SECTION 123,
    ACCORDING TO THE MAP OR PLAT THEREOF AS
    RECORDED IN PLAT BOOK 12, PAGE 1 OF THE
    PUBLIC RECORDS OF SUNNYVALE COUNTY, FLORIDA

    NATURE OF DEBTOR’S INTEREST IN PROPERTY
    Fee Simple

    CURRENT VALUE OF DEBTOR’S INTEREST IN PROPERTY, WITHOUT DEDUCTING ANY SECURED CLAIM OR EXEMPTION
    650000

    AMOUNT OF SECURED CLAIM
    600000
