Certificate of Service and Statement of Intentions

Can anyone tell me exactly what is on this document? I have done many searches, but can't find any detailed information.

Certificate of Service is a form that lists all of the creditors that were 'served' and noticed about your BK. Basically, it's the official form that tells your creditors that you have filed, and that they can participate by filing a claim.

Statement of Intentions... I'm not sure.
