Statement in Lieu of Pay Advices

Hi,

I need to file a Statement in Lieu of Pay Advices. For the entirety of 2008 I've done whatever work I can to make ends meet--it's all been 1099 work no W-2.

Any advice about what the Statement should look like?

Separatly list 12 months, income and expenses for each. You need to go into more detail for the expenses though, try to be as specific as possible. Also, make sure that the income coincides with deposits shown on your bank accounts.
