weird address on my credit report and when I should dispute

I was checking my credit report and found the following oddities
1. An address was just added and I have nothing to do with this address just that the zip code is my parents address but is in Illinois not wisconsin
Address:
BK CHPT 7 DO NOT MAIL
CLINTON,WI
2. I have two chase cards one thru bank one and its not included it shows 30 days late
a. Should i protest as it wasn't late by the day I filed?
b. It shows closed and late but doesn't show included in bankruptcy while everything else did

3. i had my 341 on oct 6th.

If you included these in BK...then yes..dispute them. They should read included in BK. As for the address. You may have to call or write the Credit reporting agency that is reporting it. I don't think you can dispute personal info on line.
