Creditor Matrix... completing and notifying reditors

1- Completing the Creditors matrix... for addresses of my creditors, simply use the address on their bill?

2- Once I file the petition with the BK court... do I notify the creditors or will the court?

Using the billing address should be fine, though they may have a 'payment' address and a 'correspondence' address.

The courts will send out noticies, within a few days normally. If there is anything in jeopardy of being garnished/seized/shut off, it wouldn't hurt for you to notify them as well-so they would know a few days sooner.

I have a question, after my discharge, do I have to notify any courts that had civil cases that the bankruptcy has been discharged?
