He's got a page to list our creditors. Does that include things like auto insurance and utilities?

Auto insurance - no. That is an "expense", rather than a creditor. Utilities are also an expense, but may ALSO be listed as a creditor if you have past-due amounts you want to have discharged in a Bankruptcy.

List cards that have a zero balance, too?

I would close the accounts that have zero balance. Just call them and have them read "closed by consumer". It is probably better than having an account read iib (included in bankruptcy).
