HMO or PPO and by state if any insurance?

When we lived in CA years back, and in AZ they majority of coverage was HMO's. In SC I found out what a PPO was and the cost of healthcare has risen drasticaly. In GA they had HMO's and more affordable again. I was paying about 50.00 for my own coverage, plus my co-pays. Here in NO, back to a PPO and it costs for both of use 250.00 per month, plus no co-pay but a deductable that is pretty high, and total out of pocket up to 11500.00 this year. So, are the larger areas still offering the cheaper HMO, or are they shifting, and what is the cost of coverage. If I just cover me my cost is still one half of the premium amount and one half of the co-pay and total out of pocket. I would like to know if the south west is changing or if this is more of a small town midwestern thing? Thanks so much.
