When everything is said and done, what does the document look like that the judge signs that says you are good to go on your Chapter 13? Does is list your payment plan and creditor information, or is it a general document?

What does the final paperwork look like?

You will receive a document from the court in the mail that states that your case is confirmed - ours says: "The relief described hereinbelow is SO ORDERED." and then is signed by our local bankruptcy court judge. This document will be posted on PACER as well.
The next section of our confirmation document states: THIS MATTER comes on for consideration of Confirming Debtors’ Chapter 13 Plan. IT IS THEREFORE BY THE COURT ORDERED that the Chapter 13 Plan is Confirmed. IT IS SO ORDERED. Then our trustee signed the document and our lawyer signed the document. That's it.
There's no listing of the monthly payment in the official court confirmation document. That came from another official document mailed to us from our trustee's office after the confirmation was granted.
I imagine every court has some kind of document very similar to ours when a Ch 13 confirmation is granted.I am not a lawyer and this is not legal advice nor a statement of the law - only a lawyer can provide those.

06/28/06 - 341 Meeting
07/18/06 - Confirmation Hearing - not confirmed, 3 objections
10/05/06 - Hearing to resolve 2 trustee objections
01/24/07 - Judge dismisses mortgage company objection
09/27/07 - Confirmed at last!
06/10/11 - Trustee confirms all payments made
08/10/11 - DISCHARGED !
10/02/11 - CASE CLOSED
