    wage garnishment form

    Does anyone know where to find the payroll deduction form that my attorney can complete to have my payments deducted by my employer?

    In my case, the judge produced the wage order form, it wasn't something my attorney did, rather it was something that came out of the BK judge's office, sent to me, which I then delivered to the people in my payroll department. Same thing when I recently got an order to suspend payments, the judge issued a suspension of payment order which I took to payroll to get the garnishments stopped for 3 months.

    Plan Modified July 8, 2009 from $1100/month to $400/month due to change in income, finally discharged in July of 2013!

      The Trustee is the one who initiates the payroll/wage deduction order. You should receive instructions from the Trustee or the attorney as to how to proceed on your end with your employer to get this accomplished. Give your attorney a call.
