I read that if your out of pocket medical expenses are more than 7.5% of your AGI, that there would be no penalty. If I needed to do this to pay for a procedure, would I need permission from the Trustee first?

The danger you have liquidating a protected asset to cash during an active 13 is that it removes the protection afforded to 401Ks when you filed. Your trustee might see whatever you take out as now-available disposable income to seize. Talk to your lawyer first and get his/her opinion about this before doing anything.
