Urgent please help. Meeting in 10 days.

My trustee is asking for these documents:

1) Sworn Testimony Form; 2) Pro Se Debtor Questionnaire

I am using upsolve.org (free non profit) since i cant afford legal advice or a lawyer. They mentioned they do not have those documents.

Does anyone know what those documents are and where I can download them so that I can email them to the trustee asap?

I would highly appreciate any help.

These are not standard bankruptcy forms. These are documents which your specific panel trustee sent to you. If you filed pro-se, you need to contact the trustee to request new copies of these documents if you lost them or never received them.
